List of requirements when applying for business permit.

New Applicants

Step A.

  1. Residence Certificate (cedula)
  2. Barangay Clearance
  3. Barangay Business Clearance
  4. 2 pcs. (2x2 photo ID)
  5. CR, OR, Driver's License(motorela, habal-habal,van for hire)
  6. Organization Clearance
  7. Location Sketch of the New Business
  8. Market Clearance(market vendors, Motorela,habal2x, van for hire)
  9. Departments of Trade and Industry (DTI) Certificate
  10. Bureau of Foods and Drugs (BFAD)
  11. National Food Authority (NFA)(Rice and corn millers)
  12. Departments of Health(DOH)License to Operate(Hospitals,Clinics)
  13. Cooperatiev Development Authority(CDA) Cooperatives
  14. Energy Regulatory Commission(ERC)(Highly combustible substances)
  15. Security Exchange Commission(SEC)(Insurance,pawnshops,courier services,banks
Step B.

After Accomplishing this requirement proceed to Mayor's Office located at the second floor of the Municipal Building for the needed forms. Next, proceed to the Treasurers Office located at the ground floor of the Municipal building for your assessment. Then after this proceed back to mayor's office for the printing of your Business , Motorela and Habal-Habal Permits.

Step B.

After Accomplishing this requirement proceed to Mayor's Office located at the second floor of the Municipal Building for the needed forms. Next, proceed to the Treasurers Office located at the ground floor of the Municipal building for your assessment. Then after this proceed back to mayor's office for the printing of your Business , Motorela and Habal-Habal Permits.

 

 Special Permits

  1. Proceed to Treasurer's office and pay for the permit fee.
  2. Proceed to the Mayor's office for the printing of permit.
